FDA Issues RF Microneedling Safety Alert: What It Means for Medical Spa Professionals

Posted By Madilyn Moeller, Thursday, November 6, 2025

The FDA’s recent safety communication regarding radiofrequency (RF) microneedling devices is a critical reminder: Aesthetic treatments involving energy-based devices are medical procedures that require appropriate oversight, training and protocols.

RF microneedling, which is sometimes marketed as collagen induction therapy, pierces the skin with tiny needles (the microneedling part) while delivering radiofrequency energy to deep tissue layers. Heating the tissues is intended to strengthen the collagen-stimulating effect for improved skin texture.

The FDA alert warns of potential for serious complications, including burns, scarring, fat loss, disfigurement and nerve damage. Some of these adverse events required surgical repair or medical intervention to treat injuries, according to the consumer alert.

While the FDA urges providers to report adverse events, it offers limited guidance on which devices, treatment areas or conditions are most at risk. This lack of clarity underscores the need for professional vigilance and structured safety standards.

What patients should know about RF microneedling

Radiofrequency is often viewed as a boosted microneedling treatment. With that added energy, however, comes added risk.

When it comes to medical treatments, leave it up to licensed, trained healthcare practitioners to decide whether you are a good candidate for RF microneedling. Seek care from a licensed health care provider with training and experience in the use of RF microneedling devices. Only a licensed healthcare professional can get you the medical attention you need if you experience a serious complication from one of these treatments.

That’s why medical spas employ medical directors, have protocols in place for proper supervision and delegation of procedures, and perform a good faith exam before putting the device in the hands of a licensed, trained and capable provider.

If you experience any problems or complications from skin procedures with RF microneedling, report them to the FDA.

Who can perform RF microneedling in a med spa?

RF Microneedling uses radio frequency energy and arrays of tiny needles to stimulate collagen growth and improve the texture of the skin. Because of this, it is considered a medical treatment and needs to be performed by or under the authority of a healthcare practitioner. What license or training is needed to perform this treatment will depend on the rules of the state you are located in.

Are you familiar with your state med spa laws and regulations? The treatment delegation table, available to AmSpa Plus Members, makes it easy to see who can perform microneedling, non-surgical skin tightening, and 21 of the most popular aesthetic treatments. In general, a physician or an appropriately trained physician assistant or advanced practice registered nurse will likely be able to provide this treatment. Those practitioners may be able to delegate the treatment to others depending on state rules. But, as always, you will want to check with your advisors and review your local rules before adding a new device or treatment to your practice.

Action items for aesthetic practitioners

AmSpa believes patient safety begins with practitioner education and accountability. RF microneedling is a medical treatment, and when it is performed without proper training or supervision, the risks are significant.

This alert also reflects a broader industry trend: Aggressive device marketing often outpaces clinical validation and practitioner readiness. From cryolipolysis to acne lasers, it is clear how inadequate training and misaligned business models can lead to poor outcomes and eroded patient trust.

AmSpa members are equipped to navigate these challenges, with extensive education, tools for practice growth, access to leading brands, thought leadership and more. Become familiar with AmSpa's:

  • Verified training programs and protocols;
  • Legal insights into FDA communications; and
  • Community committed to ethical, evidence-based care.

As aesthetic medicine evolves, so must your standards. AmSpa urges all providers to treat RF microneedling with the seriousness it deserves. Join AmSpa to stay informed, protected and prepared to deliver safe, effective care.

Let’s work together to elevate patient care in aesthetics. Because innovation should never come at the cost of responsibility.
