North Dakota Med Spa Owner Accused of Posing as Nurse During IV Treatment That Put Patient at Risk

A North Dakota medical spa owner has been arrested after police say she posed as a licensed nurse and performed medical procedures that allegedly put a patient’s life in danger.

According to the Watford City Police Department, Kendra Cain, owner of Royals MedSpa in Watford City, was taken into custody on January 25 following a multi-day investigation into her business practices. Investigators say Cain claimed to be a registered nurse and performed medical treatments despite having no nursing or medical license on record.

Police said the investigation began on January 22 after authorities received information that she was representing herself as a nurse and administering medical procedures without proper credentials. Officers confirmed through state and national databases that she did not hold any nursing or medical licenses.

During the investigation, police executed a search warrant. Inside a treatment room, officers reportedly found a used IV bag, tubing, syringes, needles, bandages, and an open sharps container. Investigators also observed what appeared to be fresh blood on the floor near the IV treatment area.

According to police, Cain admitted she had administered IV therapy to a patient using the equipment found at the spa.

The following day, officers spoke with the patient who received the IV treatment. The patient, who is a registered nurse, told police she had to “coach Cain through the IV administration process.” She also reported that the tourniquet used during the procedure was left on too long, causing her hand to turn blue and purple.

The patient told investigators she observed air traveling through the IV tubing, which she noted could result in an air embolism with the potential to cause fatal complications.

Investigators also allege Cain used the professional license of a staff member without authorization to schedule patients, prescribe medications, and carry out medical procedures when the licensed provider was not present.

Cain has been charged with multiple offenses, including reckless endangerment, unauthorized use of personal identifying information, deceptive writings, and practicing as a nurse without proper certification.
