AmSpa has updated the COVID-19 pandemic acknowledgement form in its intake forms package to include language addressing reactions to the Moderna COVID-19 vaccine in those who have dermal fillers. If you are already a Plus member, you can download the Intake Forms package for free, so you simply need to log in and download it from the AmSpa Store to receive the new form; if you have already downloaded the Intake Forms package but want the updated form, you will need to download the entire package again.
